If you are recording a DEED, QUITCLAIM DEED, EASEMENT, or LEASE; enter the total amount of the transaction. This could be the sales amount for the entire parcel or for an easement. If a lease is more than 35 years (including all options to extend), the total amount paid to the landlord for the duration of the lease should be entered.

If you are recording a document that is not a transfer of property or where there is no consideration paid, you can leave this field empty.

All documents are subject to the SB 2 fee of $75 per title - unless a valid exemption is cited. Please refer to the SB 2 Exemption Information Sheet for further details. If an exemption applies, select YES. Be sure the same exemption is cited on the first page of your document.

If no exemption is applicable, select NO. An additional $75 per title will be added.

If there is any text in your document that is more than 9 lines per vertical inch or 22 characters and spaces per horizontal inch, a $1 penalty per page shall be applied. 

Be sure that when using text boxes in PDF "shrink text to fit" is not selected. This feature often makes the text very small and illegible. The fee may be applied, or your document will be rejected due to being illegible.

If you are recording a lien, judgment or assessment and would like the County Recorder to mail a copy of the document with a notification letter to the debtor advising that a lien has been recorded, enter the total number of debtors on your document. An additional fee of $6 per debtor/notice will be added.

Please note that certain documents require a notification to be mailed to the debtor. This requirement can be satisfied by mailing the notice yourself or by having the Recorder perform this service for an additional fee.

If your document has 11-19 names to index, enter "1".

If your document has 20-29 names to index, enter "2."

If your document has 30-39 names to index, enter "3"

And so on.

If your document is an Assignment, Modification, or Termination and references more than one previously recorded document, you may be charged an additional fee.

If each referenced document refers to the same parties/names, no additional fee is charged.

If each referenced document refers to different parties/names, an additional $1 fee shall be charged for each reference that includes different parties/names to be indexed.

Check this box if you are recording a deed or lease (over 35 years) and will not be submitting a Preliminary Change of Ownership Report. A penalty of $20 will be added. 

If the completed form is submitted at the time of recording, this fee is not applied.
