Please read all instructions and requirements and complete registration form in its entirety and submit all required supporting documentation. Registrations forms must be submitted in-person by appointment ONLY beginning September 1, 2025.

All identification card photos WILL be taken in-person at the Madera County Clerk's office at the time of filing in lieu of providing passport photos. Please contact our office at (559) 675-7721 with any questions.
Effective 01/01/2022, all LDA/UDA renewal registrations must be submitted with a copy of continuing education courses completed or course transcripts. Please use the registration forms found below.
"Fillable" PDF forms are available to complete by clicking the links below. Please complete by typing in all information, print and then sign. If completing by hand, you must print clearly in blue or black ink - illegible forms may not be accepted. Additional instruction sheets are also available for your review.
LEGAL DOCUMENT ASSISTANT REGISTRATION - INDIVIDUAL: Includes instructions, fee breakdown and registration packet.
LEGAL DOCUMENT ASSISTANT REGISTRATION - CORPORATION/PARTNERSHIP: Includes instructions, fee breakdown and registration packet.
UNLAWFUL DETAINER ASSISTANT REGISTRATION - INDIVIDUAL: Includes instructions, fee breakdown and registration packet.
UNLAWFUL DETAINER ASSISTANT REGISTRATION - COPORATION/PARTNERSHIP: Includes instructions, fee breakdown and registration packet.
PROCESS SERVER REGISTRATION: Includes instructions, fee breakdown, LiveScan form and registration packet. An appointment with the Madera Sheriff's office is required for FBI/DOJ background check and is required for all new and lapsed registrations. Any felony convictions are required to submit a copy of a certificate rehabilitation, expungement or pardon along with your registration - regardless of the date of conviction.
PROFESSIONAL PHOTOCOPIER REGISTRATION: Includes instructions, fee breakdown, registration form, and employee identification card application.
* Please note that the when filing as an LDA, the use of the phrase "paralegal" in the business name may not be acceptable. Review Business and Profession Code 6450 for more information or consult a legal advisor.
Individuals who have Madera County as their principal place of business as a notary public must file their oath and official bond with the County Clerk no later than 30 days after the beginning of their commission term.
The notary may file in-person or by mail. If filing by mail, they MUST have the sworn statement completed on both copies of the oath by another notary public.
