The Madera County Department of Public Health routinely collects data for reportable communicable disease conditions, births and deaths, and factors leading to poor health.

Data Request

For additional Madera County Health data, submit a completed data request form to the Madera County Department of Public Health Epidemiologist via:

  • Online: E-Application
    • Download, complete & sign, then click submit button at top
    • Or email to dph-epi@maderacounty.com as an attachment
  • Mail: Madera County Department of Public Health Epidemiologist

  • Fax: (559) 675-0478

Standard processing is two weeks or less upon receipt of a completed data request form.  Larger projects may require addition time.
